We are an inclusive medical practice for ALL communities, ALL genders, and LGBTQAIU2+ safe & supportive.

  We hope and strive to help you feel safe and comfortable both with your visit time with us and also with your personal health decisions.  

We try to create a relaxing atmosphere by meeting patients where they‘re at in their lives.  

Our appointments are all entirely with Dr. Samuel Hawk (i.e. no nurses) therefore maximizing your time with your doctor.

Dr. Samuel Hawk


My medical practice as a Primary Care Physician and Integrated Health Practitioner was heavily focused on providing Primary Care, Hormone Replacement Therapy, Musculoskeletal Therapy, LGBTQAIU2+ services, and Gender Health Care.

I previously founded and managed two medical clinics both of which offered various services such as primary care, hormone replacement therapy (both adults & youth, all genders), legal gender correction documents, legislative advocacy for improving transgender and gender non-binary protection (anti-discrimination laws) and access to care, pre and post-op follow up care for GCS (SRS) and top surgeries, gender-affirming gynecological care, HIV management & PrEP/PEP, STD testing, counseling services and support groups. 

In addition, I've given many professional lectures, presented in conferences, participated in legislation, and trained health professionals (and organizations) on the topics of HRT, transgender & gender health care. 

Prior to entering and graduating from medical school (Midwestern University / Touro University), I completed over 2 full-time years of acupuncture and oriental medicine education. In addition, I have two Masters degrees (Chemical and Electrical Engineering). I recently moved back to Minnesota to be closer to family.  

My goal is to help you find your way to better health.  I believe in individualistic care and that YOU are the guide on your own journey - thus treatment plans will be tailored specifically to you.

Thankfully, services and acceptance have much improved since that time but still many more steps and leaps are needed.  Informed consent for HRT has always been my practice. It's unfortunate that this has not been the practice in Minnesota until recently. My goal with providing this clinic to the community is to directly help you find and follow your gender path comfortably and safely and also to improve access to care, increase local services offered, and improve our local healthcare system through inclusion, understanding, protection and non-discrimination."         Dr. Hawk



Are you struggling with fatigue, weight gain, restless sleep, low libido, irritability, sexual dysfunction, poor muscle gain or muscle loss, depression or low moods ?  Are you wanting to improve muscle strength and overall health and vitality? 

All of these may be addressed with testosterone replacement and/or supporting therapies (i.e. HCG, DHEA, estrogen blockers). We can discuss options to improve your health through herbs, exercise, life style, and/or testosterone replacement therapies (TRT). As low testosterone symptoms can be caused by many factors, we review your complete hormone panel (complete sex hormone workup, thyroid hormones, prostate, and adrenal/pituitary hormones as appropriate) and have detailed discussions about your life style and health goals.  Most patients notice improvements in the first week of starting therapy.
